When curative treatment is no longer effective
or appropriate, Hospice of the Upstate focuses on providing the best quality
of life possible. Please review the other sections of our website to
see services that we will provide to your patient.
You can continue as the patient’s primary physician under our home care
program or choose to have our Home Care Medical Director, Dr. Katie Hanna,
follow the patient. You are encouraged to stay closely involved in your
patient’s care and we will keep you informed of their status.
Hospice of the Upstate has a dedicated
Intake Department that will
take the initial referral and gather all the appropriate information.
One of our Admission Specialist will contact the family to schedule an
informative meeting and to assess the patient’s condition. Hospice of the
Upstate services can begin immediately after a care plan is determined. To
refer a patient to Hospice of the Upstate, call 864.224.3358 or fax the
referral form to 864.328.1132.
